Short-Term and Long-Term Disability coverages are available as voluntary benefit elections. The short-term disability plan offers a 14-day elimination (waiting) period, 60% of weekly salary benefit (capped at $2,000 per week), with a maximum benefit period of 26 weeks. The plan features a generous disability definition (you must have a 20% earnings loss and be limited from performing the material and substantial duties of your regular occupation in the national economy). The plan does have a 3/12 Pre-Existing Limitation, meaning any condition that existed within the three months prior to the insured’s effective date will not be covered if disability begins during the first 12 months of coverage.
The long-term disability plan offers a 180-day elimination (waiting) period, 60% of monthly salary benefit (capped at $10,000 per month). This plan also has a 3/12 Pre-Existing Limitation provision.
You are experiencing what we call an “overlapping open enrollment window.” You may need to make some decisions now (e.g., retirement elections, HSA (Health Savings Account) contributions, etc.) through your spouse/partner’s plan, but the Muir HH enrollment window allows you to make some changes, especially related to health benefits elections. More specifically:
Open enrollment for calendar year 2024 will take place the week of December 11-15, 2023 for those employees who have received and accepted an offer of employment.
Yes. Muir Home Health employees will be eligible to participate in the company’s pre-tax 401(k) Plan which includes a post-tax ROTH option, administered by Fidelity. Additional information, including information about employer match contributions, will be provided during the week of November 13-17 when the Cornerstone Benefits Team is on-site to conduct benefits education.
The 401(K) Plan offers a company matching contribution on the first 2% of compensation that you contribute at a rate of $0.25 for each dollar you contribute. You are vested in the company matching contribution at a rate of 25% per year of service, with 100% vesting after four years of service.
Muir Home Health is not permitted to provide a 403(b) plan to its employees. Employees with 403(b) plans who receive an offer of employment will be eligible to roll-over 403(b) contributions to the Muir Home Health 401(k) Plan. You can enroll immediately for the 401(K) plan. You will not be auto-enrolled in the plan.
You will want to file for the California State Disability benefit and the Unum group short term disability benefit at the same time. The Unum plan will offset the California State disability benefit from the group disability benefit. If you are still on California State disability at the time LTD begins, and you have the Unum LTD benefit, the Unum LTD benefit will also offset the California State disability benefit. Claims under the Unum STD/LTD plans will be reviewed from employee’s initial date of disability.
There is a visit limit of three (3) visits per issue, and this limit applies to in-person visits. There is unlimited telephonic support.
You will be able to view the rates (or cost of coverage) for supplemental life and supplemental AD&D coverages when you take part in the benefit enrollment experience from December 11th – December 15th. In advance of the benefit enrollment period, you can review the 953700 VLADD Flyer for rates prior to enrollment.
24/7 Virtual Visits are $0 cost, and the Virtual Visit providers are Teledoc, Amwell, Dr on Demand, Optum Virtual Care, and Walmart Virtual. If a member sees their PCP for a virtual or a telehealth visit, the copay would be PCP in-person copay.
